Under HIPAA, you have the right to ask us to correct or amend health information we maintain. Your request must be in writing and include a reason. In some cases we may not be able to make the change, for example if we did not create the information.
See our Notice of Privacy Practices for how to request an amendment. Information that came from another provider may need to be corrected by that provider.
